Print out our Paper Order Form in pdf format

Click on which order form you wish to use on the left.

2. Fill in the Order Form while you are browsing through our on-line Shop. You may wish to use the Paper Order Form to take notes of the samples or items that you wish to order. Fill in all information including the product name, and product number if applicable. Also note down the price.

3. You can Submit the On-line Order Form, OR mail or fax the Paper Order Form with your completed order to us. If you prefer to phone us, please have your completed Paper Order Form with you when you do phone, for faster service.

Please have your VISA or MasterCard ready as well.
